Acknowledgement of Unofficial Admissions Documents

You have been conditionally accepted to Columbus Tech based on "Unofficial"Admissions Documents. By completing this form you acknowledge that you are able to register for classes on the condition that you provide us your "Official" Admissions Documents by the end of the semester for which you register. A HOLD will be placed on your student account until we receive the Official Admissions Documents. The HOLD will prevent you from registering for future semesters beyond this one. Additionally, you may be responsible for any financial aid received should you be determined ineligible based on any official documents received.
